By submitting this form, I confirm that I am voluntarily enrolling in the Broadcasters & Sportscasters Mentoring Group (BSMG) mentor-apprentice broadcasting program operated by Living Waters Enterprises LLC, licensed by the California Bureau for Private Postsecondary Education (BPPE).

I understand that my training will take place one-on-one with a working broadcaster in or near my local market, and that the program length, start date, and tuition amount are as selected and acknowledged in this form.
